Position Description:
This position reports to the Program Coordinator (currently Ms. Vanessa Porter, primary report) and the Program Director (Dr. Fariba Younai, secondary report) of the Human Immunodeficiency Virus (HIV)/Acquired Immune Deficiency Syndrome (AIDS) Oral Health Care (Dental) Specialty Services Contracts with the county of Los Angeles (the "Ryan White Contract"). Patients treated under the Ryan White Contract are HIV positive or live with AIDS and require assistance at the individual level with respect to navigating through the dental care system at UCLA.

Externally, the position will be responsible for: (a) developing close working relationships with outreach workers and case managers of other HIV service providers in Los Angeles County to facilitate a referral network for general and specialty oral health services; (b) conducting initial mental health/substance abuse screenings and housing needs assessments; (c) developing and implementing a plan that addresses short and long-term housing needs, transportation, mental health/substance abuse issues, relapse prevention, medication management and any other risk factors that could impede permanent housing. Provide crisis management, case management, and other related substance abuse/mental health services to clients on the streets, in service venues, in housing placements, or other locations as appropriate; (d) implementing case management service plan goals emphasizing coordinated strategies for addressing mental health/substance abuse issues, relapse prevention, medication management and any other risk factors that could impede permanent housing; (e) assisting the Program Coordinator in making site visits to providers of oral health and medical services to Ryan White patients for the purposes of explaining and marketing SOD's oral health services; and (f) participating in organizing and supervising field rotations for students of Schools of Dentistry and Public Health in the Ryan White Oral Health Care Coordination Program.

Internally, the position will be responsible for: (a) performing psychosocial and acuity assessment on patients referred to the UCLA School of Dentistry for general dental and specialty services; (b) developing a management plan for patients that incorporates referral to Ryan White-supported psychosocial services as needed; (c) tracking patients' progress in their dental care at the School of Dentistry and to help them navigate different aspects of their care throughout the School's clinics; (d) providing administrative assistance to the Program Coordinator and Director and his/her designees, including but not limited to, accessing and analyzing patient care data and generating various computer reports; (e) learning and operating Case Watch (the software used by DHSP to manage patient-related information such as demographic data and medical status, verifying patient treatment eligibility and obtaining and reporting on patients' needs for psychosocial services; (f) learning and operating the patient care software currently used by SOD (AxiUm) and any other dental software that SOD may utilize in the future; (g) verifying the transfer of information from Case Watch into AxiUm or any other dental software that may be utilized by the SOD and vice-versa; (h) interacting directly with the Ryan White patients to provide them with assistance in understanding their treatment stages, program updates, and answer questions; and (i) participating in teaching activities at the School of Dentistry.
